Entry Instructions:

• Click Entry Form button above, complete entry form digitally, sign electronically, & save entry form to your device as a PDF file OR print form, complete form, and scan & save as a PDF file

• Complete this submission form below for STEP 1 and attach the entry form from your device then click Submit Entry Form

• Click Pay Entry Fees to complete payment for STEP 2 - entries must be paid to be accepted

• Do NOT submit payment to @olp.net PayPal account

• Do NOT submit October trial entries before entries open on 9/1/25 at 8 am CST and entries close 9/26/25 at 6 pm CST
